Course Overview
This intensive 2-day program equips professionals with a structured approach to failure management and reliability improvement, enabling organizations to move beyond reactive maintenance toward a data-driven, proactive operating model.
Participants will gain the capability to establish and sustain a fully integrated FRACAS framework, ensuring failures are systematically captured, analyzed, and eliminated leading to improved asset performance, reduced downtime, and measurable cost savings.

- Build a complete FRACAS framework from scratch
- Capture high-quality failure data (no noise, no blame)
- Apply RCA tools (5 Whys, Fishbone, FTA) correctly
- Prioritize failures using risk-based screening (RPN)
- Develop corrective actions that actually prevent recurrence
- Integrate FRACAS with CMMS / maintenance systems
- Track KPIs like recurrence rate, MTBF, action effectiveness
- Create a no-blame, learning-driven reliability culture
